The value of heart rate variability in autonomic nervous function evaluation and diagnosis of fatal familial insomnia

Background Fatal familial insomnia (FFI) is a rare autosomal dominant inherited prion disease characterized by prominent organic sleep-related impairment accompanied series of symptoms with strong clinical heterogeneity in rapid progression. As the most significant feature and potential diagnostic marker FFI, progressive sympathetic were lack objective noninvasive quantitative method to detect. Method From May 2013 August 2020, nine patients eight Creutzfeldt-jakob (CJD) normal control (NC) matched sex age recruited this study. All participants underwent nocturnal video-polysomnography included lead II electrocardiography (ECG). Heart rate variability analysis successive 5 min (in time domain methods, frequency methods non-linear measurement) was performed during quiet wake before sleep following N2, N3 REM first cycle. Result In FFI group, continuous increased heart relatively decreased amplitude oscillations compared NC CJD revealed axis equilibrium shifts towards activation. Furthermore, RMSSD HF stage significantly lower than which reflected activation parasympathetic nervous system. The abnormal LF/HF ratio SD1/SD2 combination values correlation dimension D2 group supported its unbalanced autonomic function. Conclusion Dysfunctions nerve system may be affected hypofunction partly except activation, HRV expected become diagnosis for FFI.
